1. High Fever

- Infants under 3 months: Any fever over 38°C (100.4°F) is considered serious. Immediate evaluation is needed.
- Children over 3 months: If fever lasts more than 3 days, or goes above 39°C (102.2°F), consult your paediatrician.
- Fever with alarming symptoms: Rash, stiff neck, seizures, or excessive drowsiness are red flags.
2. Difficulty Breathing
Breathing issues can range from mild to life-threatening. If your child is having trouble breathing, it’s best to get medical help immediately.
Signs to watch for:
- Rapid or laboured breathing
- Wheezing, grunting, or flaring nostrils
- Persistent cough or bluish lips or fingers

3. Persistent Vomiting or Diarrhoea
Children can occasionally experience upset stomachs, but persistent symptoms could lead to dehydration or point to infections such as gastroenteritis.
Seek medical attention if:
- Vomiting lasts more than 24 hours
- Diarrhoea continues for more than 2 days
- Signs of dehydration: dry mouth, sunken eyes, no tears, or less than 3 wet diapers per day
4. Excessive Sleepiness or Lethargy
A sick child may naturally be less energetic, but excessive sleepiness or lack of responsiveness could signal something more serious.
Warning signs:
- Difficult to wake or keep awake
- Limp, floppy body
- Not responding to sounds or touch
Lethargy in children can be a sign of serious infections, neurological issues, or metabolic problems.
5. Seizures or Sudden Jerky Movements
Seizures can be alarming and are always a reason to seek immediate medical attention, especially if:
- It’s your child’s first seizure
- The seizure lasts more than 5 minutes
- There is loss of consciousness, bluish lips, or rigid body movements
Febrile seizures (related to fever) are common in children under 5, but still warrant evaluation by a qualified paediatrician.
6. Poor Feeding in Infants
Feeding difficulties are particularly concerning in newborns and infants under 6 months.
See a doctor if:
- Your baby refuses to breastfeed or bottle-feed
- There is weight loss or poor weight gain
- Your baby has fewer than 3 wet diapers a day
Poor feeding could be linked to infections, reflux, or congenital issues, and needs a prompt assessment.
7. Rashes That Spread Quickly or Don’t Fade
Skin rashes are common in children, but certain types need medical attention:
- Rash accompanied by fever
- Purple or red spots that don’t disappear when pressed
- Painful, swollen, or rapidly spreading rashes
These could indicate meningitis, viral infections, or allergic reactions.
8. Ear Pain or Tugging at the Ear
Ear infections are common in babies and toddlers and often follow a cold or respiratory illness.
Watch for:
- Frequent crying or irritability
- Tugging or rubbing the ear
- Trouble hearing or responding to sounds
Untreated ear infections can lead to complications, so a visit to your paediatrician is important.
9. Persistent Pain or Swelling
If your child complains of ongoing pain — in the limbs, joints, or abdomen — that doesn’t improve with rest or over-the-counter medication, consult a doctor.
Also watch for:
- Swelling in joints or limbs
- Limping or refusal to walk
Persistent pain could point to injuries, juvenile arthritis, or infections such as osteomyelitis.
10. Sudden Behavioural Changes
Mental and emotional well-being are equally important. Changes in behaviour can sometimes signal neurological, emotional, or developmental issues.
Seek help if you notice:
- Sudden aggression, confusion, or extreme withdrawal
- Loss of speech, walking, or other previously learned skills
Early intervention makes a significant difference, especially for conditions like autism spectrum disorders or ADHD.
